Remember:
- Bills for care outside of the Health Center are the responsibility of the student.
- Student Insurance Claim forms and assistance completing them can be obtained from the Health Center.
- Students must submit the claim form and bills to the insurance company.
Periodically, Nursing and Sports Medicine students will observe in the Health Center. ALL CHARTS ARE KEPT CONFIDENTIAL. However, if you are more comfortable not having a student participate in your care, just tell the staff when you check in. Information will only be released with a signed form from the student.
